Complete and print or print and complete the order form. With the completed form and your credit card available call in you order to the number at the bottom of the form.
ORDER FORM IN MS WORD FORMAT
Click above on "ORDER FORM" to open the order form in
MS WORD format. Save the blank form to your computer. Open the form and complete the information. Use "save as" in word to save the completed form with your name and "order" as the file name. Attach the word document to an e-mail with "Order from" and your name and the name of your church in the "Subject".
